MBNA reply to 2nd letter

I have received a reply from MBNA ( 2nd letter)
'We are in receipt of your letter dated 8 October 2011 in relation to the above account.

We recently responded to your comments on 30 September 2011. A further copy of our letter is enclosed.

As it would be disproportionate to the issues raised, we do not intend to enter into subsequent correspondence with you about these issues and this is our final response on the matter.'

They go on to suggest I may want to note the CCCS debt remedy and tel no's for other debt management/ advice.
The letter was addressed to me in lower case..is this significant?
What is my next step, any help & advice is much appreciated
