example stamp and seal

Can anyone tell me if the example I have attached of the stamp/seal is all that is required, or does anything need to be added.
There appears to be a lot of uncertainty, and so have uploaded one for approval. ( obviously, details have been changed for the purpose of this example)

sorry, it's a bit sideways.

u will need your signiture in red ink at a slight angle runnig on to the stamp

Are you using a postage stamp also? is that compulsary or does it bear some underlying meaning in law?

Is that thumbprint and signature on top of postage stamp, or stamp over thumbprint and signature?

on top of the stamp, signiture starts on paper and runs onto stamp just to make sure its not removed
